Last week, a U.S. Food and Drug Administration advisory panel voted to recommend broader access to several unapproved peptides. The Pharmacy Compounding Advisory Committee voted in favor of expanding access to the peptides BPC-157, KPV, TB-500, MOTS-c, epitalon and semax. The votes came amid a surge of public interest in these unapproved therapies, many of them heavily promoted on social media. FDA scientists objected throughout the meeting, citing a lack of safety evidence and, in several cases, uncertainty over the compounds’ precise chemical structure.
The recommendations are non-binding, but the FDA often follows the advice of its advisory committees, potentially allowing compounding pharmacies to produce these peptides for prescription use nationwide.
Does the hype outpace the evidence?
The panel’s decision comes amid a wellness trend driven largely by social media. Fitness influencers with millions of followers increasingly present peptides as self-improvement tools, using dramatic before-and-after photos and claims that the products boost energy, recovery, physique and athletic performance. Many promote the products without adequately disclosing commercial relationships or the associated health risks.
The scientific evidence tells a very different story. For BPC-157 in particular, most published research has been conducted in animals, with no Phase 1 human safety trials or controlled studies demonstrating effectiveness in people. Despite widespread promotion, no human clinical trial has shown BPC-157 to be effective at repairing muscle injuries or improving athletic performance in healthy individuals.
One physician-researcher told NPR that clinical trials remain the only reliable way to determine both risks and benefits. The expert also noted that dosing protocols are often arbitrary, with little scientific rationale behind the amounts users inject. Because much of the peptide market operates online, consumers frequently have no way of knowing whether products are contaminated or accurately labeled, while the underlying safety data remains limited. Reported adverse effects include gastrointestinal problems, headaches, fatigue, allergic reactions and infections.
Selling peptides labeled “for research use only, not for human consumption” is less a genuine safety warning than a legal shield, allowing vendors to distance themselves from therapeutic claims while continuing to market products that many customers ultimately use for those purposes.
None of it changes the anti-doping rules
For competitive cyclists, easier access through U.S. pharmacies changes nothing. BPC-157, TB-500 and related compounds remain prohibited at all times by the World Anti-Doping Agency (WADA) and National Anti-Doping Organizations (NADOs), regardless of whether they are legally prescribed, commercially available or endorsed by the FDA.
The endurance sports community has already seen the consequences of confusing “legal to buy” with “legal to race on.” Last September, the United States Anti-Doping Agency banned age-group triathlete and coach Anthony McCauley for four years after a whistleblower revealed he had used and promoted BPC-157, TB-500, CJC-1295 and ipamorelin to athletes competing under the WADA Code. The sanction centered on anti-doping rule violations involving complicity and administration, not simply personal use.
Broader government moves to expand access to peptides, combined with aggressive promotion by social media influencers, are likely to increase the number of amateur cyclists, gran fondo participants and masters competitors using them.
WADA and national anti-doping organizations are ready. Athletes who mistake easier access for legal use in competition risk lengthy suspensions and being branded as dopers, regardless of how the peptides were obtained.
